Student File Maintenance
All students in the Physical Therapist Assistant Program will have an official student file that is maintained in a locked cabinet in the office of the Program Director. This file will include, but is not limited to; the program application form, correspondence between the student and the program, copies of transcripts, degree outlines and summaries as they are completed, student exams, clinical performance records, advising, any disciplinary actions or documentation, and student comprehensive examinations. A student will not have direct access to their personal files, however, the student may request to review the file with the DCE or PD. All files, including documentation of student’s course work are maintained for 2 years, then scanned into the student’s electronic file. The hard copy of the student’s file will be destroyed in order to satisfy institutional accreditation requirements.
